Personally I don't see a problem in most names actually.

I think I didn’t express myself well and would add to what I had to say.

So, our man has a nickname that I doubt he would apply to himself, and if he complains about something, regardless of whether he is right, or not, that nick will show up in our face, feels like an offense.

The insulting guy, should get a mute than.
The other, well if it's offending we can ask to change his nick, we also can autokick certain nicks.

Personnaly I don't realy care about any insulting names or chats ingame. But if it's for the peace of the server, I will also act when I see it, or when someone if offended.
